Patients
If you are a participant of the National Disability Insurance Scheme (NDIS) and you need any help to access information about your plan, need a change in your NDIS funding or need help to access disability support services whilst you are in hospital, we can help. Additionally, if you are not a participant of the NDIS, but think you might be eligible, we can provide information about the access process, and support you through this if required.
We take a team approach to your care, with each member having a vital role in supporting you. Your ward social worker can assist you in a variety of ways, such as supporting you to access the NDIS, advocating for increased NDIS funding, referring you to service providers or linking you with a disability advocate. You can ask your ward for a referral to a social worker or contact us directly on 9400 9145.
If your query is specifically about equipment (or “assistive technology”) you will need to speak to the relevant member of the allied health team, such as an occupational therapist or physiotherapist. You can ask anyone on your ward about accessing this service.
We also have a team of specialised nursing staff who you may want to speak to during your stay, such as the diabetes educator, continence nurse or wound care specialist. You can ask your doctor, nurse or any of your allied health team about this.

Families and carers
If you are a family member or carer, you can ask to speak with the treating team, nursing staff or a member of the ward’s allied health team. Remember that we cannot share patient information without consent, but we can provide general information and direction on services and support options. Where consent is given we will include you in discussions about treatment options and discharge planning. If you have a disability and need assistance to attend these discussions, please let us know in advance.
Service providers
If you are a service provider and you have a query about an NDIS participant who is a current inpatient of Joondalup Health Campus, please contact the NDIS team via our email: NDIS.JHC@ramsayhealth.com.au and we will respond to you as soon as possible during business hours.
If your query is more urgent you can contact Joondalup Health Campus on 9400 9400 and ask to speak with the NDIS social worker on the roving phone number 5129.
We are only able to provide support to current inpatients, or previous patients who are at immediate risk of readmission. All other participants should be directed back to their support coordinator or local area coordinator (LAC) partner.
Our NDIS team is available Monday to Friday 8am – 4pm.
